Overview of AssessNET (Reporting System)
Since January 2020, the University has been using AssessNET as our online accident/incident reporting system. Paper forms (or scans/emails of the old forms) are no longer available or accepted.
All accidents, however trivial they may seem, should be reported to the Safety Office in the first instance. Investigations may follow and will be added to the initial report.
If you think an incident falls under one of the RIDDOR reportable categories, advise the Safety Office without delay. To comply with the law the Safety Office must report these to the Health and Safety Executive. For more information on RIDDOR, please see the link in the menu to the left of this page.

AssessNET has two different levels of access: a Registered User can log in to the full AssessNET system to process, modify and work on accident records. This type of user will see all accidents that occur within the incident centre. Examples of Registered Users are Departmental Safety Officers (DSOs), Departmental Administrators (DAs) or other designated staff.
The other level is a ‘Portal User’: these are individuals who can submit an initial accident record, but once submitted, they can’t modify the record or carry out the completion of the record. This type of user is often a First Aider, the injured person (IP) or a witness/reporter.
NOTE TO ALL REGISTERED USERS:
Registered Users must enter their AssessNET password to access accidents/incidents on AssessNET.
Most departments have at least two people registered to submit online accidents through the main online reporting system. These are usually the DSO and the DA or a deputy DSO. If you are aware of any changes to personnel, please email us immediately so we can update our contact list at accidents-incidents@admin.cam.ac.uk.
All those registered as users will be sent an ‘activation email’ from AssessNET and have to activate their accounts before being able to use the main system. In December 2022 there was a change to the login procedure for AssessNET. AssessNET/Riskex has issued guidance and information: 'New AssessNET login information.'
Enter your registered email address within the AssessNET software, then use your AssessNET password, followed by your chosen 6-digit PIN code to access Accidents/Incidents on AssessNET.
NOTE TO ALL PORTAL USERS:
AssessNET is a cloud-based system and has many security features to keep our information safe and confidential. The Portal can only be accessed by one of two means. Either via the 'Accidents and Incidents' webpages of the Safety Office website (accessible to anyone with a University account, formerly known as 'Raven'). Alternatively, the Portal user can use a 2-D barcode featured on departmental incident reporting posters and leaflets. A smartphone can then be used to reach the PORTAL. Portal users do not need to register to log into AssessNET.

It is not possible for a member of the public to log an incident as they do not have access to the Safety Office webpages.
Portal users can access AssessNet only by using a University computer or, if working from home, accessing it via the University’s remote access area using the internet from within the firewall. For members of the University without access to a computer

Once they have completed their section of the relevant form, the reporter of the incident should pass the completed form to their line manager or to the relevant DSO for inputting the information online.
What happens when an incident record has been submitted?
Once an incident has been submitted on AssessNET the DSO will receive an email from AssessNET, informing them that a new incident has been reported and requires their attention. When the incident record has been completed by the DSO the Safety Office will check for correctness and completeness before it is electronically signed off.
