Investigating Accidents

Most accidents will not need a very detailed investigation. All work-related accidents/incidents need the completion of the ‘Investigation’ section as the minimum. Please do not remove the ‘Investigation’ section from the incident record menu.

There will be some instances where statements, photographs, etc will be of great benefit in determining the immediate and root causes of the accident. These causes will need to be dealt with to modify the risk assessments and improve controls.
